Need to make a claim?

We are sorry to hear that your pet is unwell. We make claiming on your policy as easy as possible, simply follow the steps below.

Steps to making a claim

1. Download a claim form by clicking on the link below

2. Fill out your claim form and sign page two

3. Your vets need to complete their section on the claim form and sign/stamp page two

4. Submit your fully completed claim form (ensuring all boxes are completed) to us to by email at ClaimForm@perfectpetinsurance.co.uk, along with the treatment invoice and the full clinical history

The full clinical history consists of all available medical notes since your pet was born, including clinical notes around all vaccinations

We will need any clinical notes that are also available before your pet was in your possession, for example, a breeder or previous owner, or whether you have taken your pet to different vets

If this is not submitted at time of claim submission, it will delay your claim being processed

Can Perfect Pet pay my vet directly?

We can pay your vet directly (at the discretion of your vet). Simply ask your vet to fill in the payment details section on page two with their details.

How will I know if you have paid my vet?

You will receive a remittance breaking down your claim.

What should I do if I am unhappy with the outcome of my claim?

If you are unhappy with the outcome of your claim, please contact us on our dedicated customer contact number 01992 667 330.

How will my claim be paid?

We pay claims by BACS transfer.
If you pay by Direct Debit, we will pay into the same account we take payment from. If you pay by another method, you will need to nominate an account on your claim form and provide details.

If you choose to have your vet paid directly your vet will need to provide their payment details on page two of the claim form.

How long do I have to make a claim after treatment?

All claim submissions must be made within 90 days of the date of first treatment.
